The High Achievers' Wellness Reset Program Agreement

This Agreement is made today between Epicurean Element, LLC dba She Wellthy (herein referred to as She Wellthy) and the client.

I understand that I am participating in an 8- week group coaching program.


This means that through an interactive video connection via Zoom, I will be able to see, listen to, and/or speak with Ivelise 'Eva' McLaurin the health coach provider alongside other people who may be present in the group coaching session.


I understand there are potential risks with this technology:

  • The video connection may not work, or it may stop working during the coaching session.

  • The video picture or information transmitted may not be clear enough to be useful for the coaching session.

  • Video recordings will be providing within 48 hours into my member portal each week.


I also understand other participants will be attending the group coaching session and that they will take reasonable steps to maintain confidentiality of the information obtained during each session.  I have read this document and I hereby consent to participate in group coaching sessions under the conditions described in this document.


CONFIDENTIALITY:

  • Anything said between any two or more group members at any time is part of the group and is confidential​.​ I understand that everything said in this group is confidential and not to be shared with anyone outside of the group, except as may be otherwise required by law.

  • I agree to keep confidential the names of other members of the group and what is said in the group. As a member of this group, I agree to not disclose to anyone outside the group any information that may identify another group member. This includes, but is not limited to, names, physical descriptions, biological information, and specifics to the content of interactions with other group members.

  • I agree to indemnify and hold Epicurean Element, LLC DBA She Wellthy harmless for any loss or damages, including costs and attorney’s fees, incurred by Epicurean Element, LLC DBA She Wellthy as a result of my breach of another’s confidentiality.

  • I understand that efforts to keep all information confidential are made, and likewise, I will take steps to safeguard my member account login information. I will not share my login with anyone or give them access to my account or virtual sessions.


SERVICES:  Coaching is a relationship of respect between She Wellthy and the Client. She Wellthy & Client will co-explore the topics, challenges, concerns, changes and successes of health & wellness in this integrative, holistic, and skilled group coaching program. The approach will be to utilize Client's strengths and values with coach strategies to move toward greater health and wellness. The Program in which you have enrolled in will include all of the following: 

A.  Upon payment received, I will create an account to sign up to member portal to access program modules, companion materials and a wellness assessment to fill out upon purchasing program.

B.  (1) 90-minute zoom video group coaching sessions to provide an overview of program.

C. Seven (7) 60-minute zoom video group coaching sessions (commencing one (1) each week) to review progress and adjust goals as necessary.

D. (1) 90-minute zoom video group coaching sessions to conclude the program, group discussion on progress and future health and wellness goals.

E.  Access to a private member portal to access all modules, future recordings, handouts and course materials.  This platform will serve as your single point of contact with your Health Coach.

F.  Email support in between sessions for questions, clarifications and accountability.

G.  Complementary supporting materials (hard copy and digitized form) may include recipes, books, hand-outs, etc.,.  Customized resources upon request to set you up for success such as: assignments, handouts, worksheets, workbooks, checklists, practices to complete in between sessions and recommended reading related to health and wellness. Any supporting material either digital or hard copy provided, is for personal use only.  She Wellthy maintains IP ownership of supporting materials. 

H.  Tools (such as breathing exercises, coping mechanisms, etc.,) may be provided to empower exploration of new strategies & paths to health. She Wellthy may, in its sole discretion, provide the Client with tools and techniques to support their journey.
